Hunger after stopping GLP-1: why it comes back and how to handle it

Hunger after stopping a GLP-1 medication is normal: the drug was turning appetite down, and as it leaves your body over roughly 4–6 weeks, hunger, fullness and "food noise" drift back toward where they were before you started. For many people it feels stronger than before, because the body also pushes back after weight loss. You can't switch it off, but you can plan for it: eat on a clock, put protein and fibre in every meal, plan for your hungry hour, sleep, keep training, and tell your prescriber early if eating starts to feel out of control.

Below: when hunger returns for each medication, why it can feel worse than before, how to tell body hunger from food noise, a day of eating built for big appetite, an evening plan, and when to get help.

When does hunger come back after the last dose?

It comes back gradually, as the drug level falls. A useful rule from pharmacology: a medicine is mostly gone after about five half-lives.

MedicationHalf-lifeMostly cleared afterWhat people usually notice
Semaglutide weekly (Ozempic, Wegovy)About 1 weekAbout 5–6 weeksAppetite starts creeping back in the first 1–3 weeks; most of the effect gone by week 5–6
Tirzepatide weekly (Mounjaro, Zepbound)About 5 daysAbout 4 weeksA similar pattern, a little faster
Liraglutide daily (Saxenda, Victoza)About 13 hoursAbout 3 daysAppetite returns within days
Oral semaglutide (Rybelsus)About 1 weekAbout 5–6 weeksLike the weekly shot

Half-lives come from the prescribing information. The Wegovy label, for example, asks women to stop at least 2 months before a planned pregnancy because of its long washout. How it feels varies a lot: some people notice hunger within days of a missed dose, others not for a month. If you taper with your prescriber, the return is spread over the weeks of the taper instead.

Weight follows. Houston Methodist cites a 2025 study in BMC Medicine finding that meaningful regain can start as early as 8 weeks after stopping, and our guide on weight gain after coming off GLP-1 has the trial numbers for the first year.

Why it can feel worse than before you started

None of this is a lack of willpower. It's also not withdrawal in the drug sense: GLP-1 medications aren't considered addictive. Clinicians describe it as losing the benefit, not a rebound reaction.

Body hunger or food noise? Tell them apart

"Food noise" is the constant background thinking about food that many people said went quiet on the medication. It often returns before real hunger does. They need different answers, so it helps to know which one you're facing.

Use a 1–10 hunger scale

Before each meal for the first 8 weeks off, rate your hunger from 1 (sick-hungry) to 10 (painfully full). Aim to start eating around 3–4 and stop around 6–7, comfortably satisfied. Write the number down next to the time. In two weeks you'll see your pattern: the hours when you get to 2 before you eat, and the meals that don't hold you. That tells you where to put a snack and which meals need more protein or fibre.

What to eat when hunger comes back

The aim is not to eat as little as possible. It's to make real hunger meet food that keeps you full for longer, so you aren't fighting your appetite all day.

Protein first, at every meal

Protein is the most filling of the three macronutrients in most studies (Leidy and colleagues reviewed them in the American Journal of Clinical Nutrition, 2015). Keep the target you had on the medication; our guide on how much protein to eat on GLP-1 explains the 1.2–1.6 g per kg range. A working amount is 25–35 g a meal: 4 oz chicken or fish, 1 cup Greek yogurt, 3 eggs with a slice of cheese, or 1 cup cottage cheese.

Fibre and volume

Foods with a lot of water and fibre fill the stomach for few calories: vegetables, salads, broth soups, fruit, beans and lentils, oats. Studies by Barbara Rolls at Penn State found that starting a meal with a low-calorie salad or soup led people to eat less over the whole meal. The US Dietary Guidelines suggest about 14 g of fibre per 1,000 calories, which is 25–30 g a day for most adults. Build up over 2–3 weeks to avoid bloating.

Slow carbs instead of fast ones

Pick whole-grain bread, oats, potatoes with the skin, brown rice and beans over white bread, pastries and sweet drinks. They take longer to digest and, with protein, smooth out the dip in energy that can feel like hunger an hour after eating.

Eat slowly

Fullness signals take time to reach the brain. In a small study in the Journal of the American Dietetic Association (Andrade, 2008), women ate less at a slow meal than a fast one and felt as satisfied. Put the fork down between bites, and give a meal 20 minutes before deciding on seconds.

Drinks

Water with meals and through the day; thirst is easy to read as hunger. Go easy on alcohol: it lowers your guard on food, and many people find a drink is what starts an evening of snacking.

A day of eating built for a returning appetite

Regular meals stop hunger from building to the point where it wins. Harvard Health suggests eating every 3–5 hours when coming off a GLP-1. Here is an example day at about 125 g of protein and 30 g of fibre; scale portions to your own needs.

Values are typical (USDA FoodData Central). If you'd like a week of protein-first meals with a grocery list, our GLP-1 meal plan for beginners works off the medication too: just let portions grow with your appetite.

The evening plan

For many people, the hardest hours off the medication are after dinner. Plan them like a meal.

  1. Make dinner big enough. A dinner with 30–40 g of protein and a pile of vegetables leaves less room for grazing later.
  2. Decide the snack in daylight. If you know you'll want something at 21:00, choose it at lunch: yogurt, fruit, a protein shake, popcorn. Eat it at a table, from a bowl.
  3. Close the kitchen. Brush your teeth after the snack. Many people find it a clear "done" signal.
  4. Move the trigger. If TV is when you snack, keep your hands busy or watch somewhere other than the sofa next to the kitchen.
  5. Keep easy-to-overeat foods out of sight. Not banned. Just not on the counter, and bought in single portions.

Sleep, stress and training change your appetite too

Start before the last dose

The best time to build the habits above is while the medication still keeps hunger low, when small changes take little effort. If you and your prescriber have set a date:

Whether to taper, and how, is your prescriber's decision. Never change or stop a dose on your own.

When to call your prescriber

Common mistakes

  1. Skipping meals to "save" calories. The hunger you push back at noon returns twice as strong at 21:00.
  2. Treating every craving as hunger. Check the scale and the table above first.
  3. Treating hunger as failure. It's your body doing what bodies do after weight loss. Plan for it.
  4. Dropping protein as appetite grows. It's tempting to fill up on fast food. Protein holds you longer and protects your muscle.
  5. A crash diet to beat it. Very low-calorie plans after stopping tend to end in bigger swings.
  6. Waiting until it's bad. The habits are easiest to set while you're still on the medication.

Questions people ask

How long does extreme hunger last after stopping Ozempic or Wegovy?

There's no measured answer. Appetite usually settles back toward where it was before you started, not above it, once the drug is gone (roughly 5–6 weeks for weekly semaglutide). The body's push after weight loss can keep hunger higher for a year or more, which is why the habits matter long term.

Is the hunger worse after Mounjaro or Zepbound?

No study compares them directly for hunger after stopping. Tirzepatide clears a little faster, so appetite may return a little sooner. SURMOUNT-4 showed meaningful weight regain within a year after switching to placebo.

Does food noise always come back?

Many people report it does, at least in part. Some find it quieter than before the medication, especially if they built new routines. A plan for your triggers helps either way.

Can protein shakes help with hunger after stopping?

A shake with 20–30 g of protein is a useful planned snack. Our guide to protein shakes on GLP-1 covers what to look for on a label. Food that you chew tends to fill you up more than a drink of the same calories, so use shakes for convenience, not every meal.

Should I just restart the medication?

That's a decision for you and your prescriber. Some people restart, some switch medicines, some manage with habits alone. It helps to agree in advance on what would make you come back in.
